Hop Up Inspection Checklist

Use this hop up inspection template to assess portable platform access equipment on site. Check platform integrity for bending, dents, cracks, corrosion, or contamination; stiles for warping or damage; hinges for security, movement, and rust; feet and caps for condition and fit; and hooks and catches for presence and security. Capture photos of the unit and tag, record location and ID, document comments and an overall result, and sign off with inspector and user signatures.

Record site details, subcontractor and preparer, list operators using hop ups, and specify the exact location of use. Confirm the 600 x 600 hop up and note if safer means of access are available, including why alternatives are not practical.
