This quarterly inspection form helps competent persons evaluate the condition and compliance of full body harnesses and lanyards. Capture asset details including make, model, serial and batch numbers, dates of manufacture and in-service, and maximum body mass. Verify stitching and webbing integrity, check hardware such as D-rings and buckles for corrosion, cracks, distortion, and proper movement, and assess hooks for alignment and safe closure. Confirm labels and serial numbers are legible, review CSA class suitability, and record the time of last inspection. Add notes and photos, set pass or fail disposition, and remove unsafe equipment from service. Supports OSHA and WHS fall protection requirements.
