Use this Electrical Shock Incident Report to document and investigate electrical shock events. Capture date and time, job and work order numbers, location, and the person affected. Record whether medical attention was advised, how the shock occurred, faults or defects identified, remedial actions taken, and any further works required. Note if Western Power was notified and include a reference number. A companion Shock Incident Test Report sheet lets you log supply voltage at the switchboard, confirm MEN link, measure earth resistance, voltage to independent earth, and point-of-shock voltage. Verify RCD protection and socket polarity, and perform line impedance, RCD trip time, insulation resistance, and fault loop tests. For shocks from taps or pipes, add continuity to water pipes, voltage under load, and comparative voltages with the main switch off. Complete with tradesman details, signatures, and final sign-off to support safety and compliance.
