This CLH Infraction Notice template helps construction teams document site violations and formal warnings. Record site and PCBU details, select the type of warning such as 1 week notice, 2 weeks notice, or cease work, and specify offenses including no current White Card, substandard work, SDS not current, violation of safety rules or company policies, and tag and test out of date. Capture incident details, outline a plan for improvement, set consequences for further infractions, and obtain acknowledgement via signatures from the site supervisor, PCBU, and a witness. Use it to assign responsibility, set deadlines, and track follow up actions to strengthen safety and compliance on construction projects.
