This CHOP Suspended Scaffold checklist helps crews verify swing stage safety before work begins. Inspect the scaffold rig, including secured counterweights, tie backs, intact wire mesh and debris trays, and emergency descent function. Confirm all crew are trained and equipped with radios for clear communication. Check fall protection: lifelines, retractables, and rope grabs are defect-free and anchored to engineered points, edges are protected, lifelines use proper knots or termination bars, and guardrails, warning lines, or PFAS are in place at leading edges. Ensure tools and materials are tethered, small items stored in zipped bags, and transfers between roof and basket are controlled. Establish a Controlled Access Zone with hard barriers and signage, assign a groundman with radio, and complete required documentation including the daily log book and swing-specific PTP.
