Dit Aditech formulier ondersteunt het registreren en opvolgen van ongevallen, bijna ongevallen, gevaarlijke gebeurtenissen en calamiteiten. Het legt kerngegevens vast zoals datum, locatie, project, leidinggevende en slachtoffergegevens. Het bevat velden voor type incident, meerdere slachtoffers, behandeling door eerste hulp, arts, bedrijfs hulpverlening of ziekenhuis, en verwachte duur van verzuim.
