Deze Nederlandse template ondersteunt de aanvraag voor keuken wrappen bij renovaties. Inspecteurs registreren adresgegevens, reden van aanvraag, lengte en samenstelling van het keukenblok, leeftijd van de keuken en de staat van boven- en onderkasten. Er zijn velden om schade en slijtage te noteren zoals opgezwollen frontjes, beschadigde of verbrande zijwanden, versleten scharnieren, slechte ladegeleiders en plinten. De ondergrond van de kasten, de conditie van het werkblad, kit, tegelstrip, spoelbak en kraan worden beoordeeld, inclusief de vraag of vervanging van het blad nodig is en de afmetingen daarvan. Ook worden ZAV elementen, hoekopstelling, open kasten, kleurkeuze, handgreep vervanging en voorkeur voor uitvoeringsdagen vastgelegd. Foto-uploads voor keuken, asbestverdacht materiaal en specifieke schades zijn voorzien, evenals handtekeningen van inspecteur en bewoner.
