Operator Vehicle and Driver Observation Report

Use this checklist to assess vehicle condition and driver performance against standards. Record vehicle details, class, seats, and accessibility features such as wheelchair access, air conditioning, wifi, navigation, and tracking. Inspect exterior and interior items including lights, tyres, windows, cleanliness, safety equipment, and seatbelts. Evaluate driver presentation and behaviors, route knowledge, assistance to passengers, announcements, and compliance with policies and tachograph records. Add comments, capture photos as evidence, and obtain signatures to complete the observation.
