This maritime template provides a structured grounding report and procedures checklist for vessel Masters and crews. Capture key details after a grounding incident, including vessel name, exact position or river mile, course and speed, water depth soundings, current conditions, and the nature of the bottom. Record tank fluid levels, assess main engine and equipment status, and document whether the vessel can be moved without assistance or is in an exposed position. Use the form to guide immediate reporting to the Lead Master or Designated Person and ensure a complete, consistent incident record with a Master signature.
