Crew and Vehicle Observation Report Checklist

This crew and vehicle observation report helps supervisors assess driver behavior, vehicle condition, and personal safety compliance during operations. It covers DVIR completion, safe driving techniques, signaling, parking, backing, and traffic law adherence. Vehicle checks include lights, alarms, mirrors, cameras, tires, reflective tape, fire extinguishers, and registration. Personal safety covers PPE such as hard hats, steel‑toe boots, ANSI hi‑vis garments, and safety glasses, plus proper lifting and container handling. The checklist also verifies lockout/tagout practices, wheel chocking, and corrective actions with signatures and photos.
