This employee observation report template helps supervisors evaluate heavy equipment operators on safety, skills, and compliance. It covers DVIR completion, training and recertification, equipment condition checks, operating skills such as travel speed, braking, load carrying, and control use, and personal safety requirements including PPE and safe mounting and dismounting.
