This Bendigo SES Event Reporting template helps capture critical details from emergency responses. Record CAD event numbers, job type, number of casualties, and track extrication start and completion for multiple casualties. Log vehicles involved and details for each, note the VicPol commander, on‑scene and clear times, and document the job story. Capture vehicle and crew information, code travelled, one‑way kilometers, command roles such as driver, SES commander, and safety officer, as well as chainsaw or pole saw usage and members operating equipment. Includes sections for additional SES vehicles on scene and the member completing the report. This template is for reference only and is not linked to the official VicSES IRS; reports must still be phoned in or entered into the IRS.
