121 ARW Unit Safety Representative Assessment FY23

Use this biannual safety representative assessment to evaluate unit mishap prevention effectiveness and compliance across the 121 ARW. The checklist covers twelve month mishap summaries, management and supervisory support, outstanding performers, and detailed unit safety representative program requirements in line with Air Force guidance. Verify risk management training, spot inspections, mishap procedures, safety education, and hazard abatement. Record findings with probability, severity and risk codes, document out brief steps, and send a formal report to commanders.
