121 ARW MHE Program Safety Assessment FY23

This template supports a comprehensive biannual safety program assessment for Material Handling Equipment used by the 121 ARW. It aligns with Department of the Air Force guidance including DAFMAN 91-203 and AFI 91-203, covering mishap prevention effectiveness, compliance checks, and documentation. Sections guide prior to use, frequent, and periodic inspections for hoists, cranes, slings, and powered industrial trucks, verification of certifications, and record keeping. It also includes training and qualification requirements for instructors and operators, mishap summaries, findings with Risk Assessment Codes, causes analysis, and a formal out brief and reporting process to strengthen unit readiness and risk control.
