This Uniti Fiber Install Report template helps technicians document fiber installations at customer premises. Record order number, install date, installer details, CPE name and address, gear location, asset tags and serial numbers, carrier type, and any gate code or lock combinations. Capture power configuration (AC, DC -48, DC +24), ground readings, and add photos of the install. Log optical readings, including RX at LGX, TX at equipment, and RX at equipment. Finish with a completion signature to confirm successful installation and handover.
