Use this close-out template to confirm telecom site readiness and document every stage of fiber construction. Capture facility and trench or bore photos, hand hole and fiber installation images, dig ticket and utility locate evidence, and marked-up as-builts. Record inventory such as reel numbers, fiber and conduit footage, hand holes, tracer wire, NEMA enclosures, and strand. Note customer actions, add special remarks, and attach additional instructions. Finish with auditor signature for a complete, compliant handover.
