This NGA High Cost Approval Form (V3) is used to document and approve telecommunications network remediation work. It captures S/O number, address, dates, preparer, scope details, quotes, adherence to the NGA lead-in decision tree, photos and concept drawings, network and duct sizes, civil works inside and outside boundary, code claims, network changes, GPS coordinates, and site diagrams. It also records Chorus requirements, UN ticket numbers, exchange area and FFP numbers. On completion, submit to your field manager and the designated Chorus and Visionstream email addresses for review and approval.
