This gym leisure facilities inspection template helps teams perform monthly hazard checks across the venue. Review structural and facility items such as doors, walls, windows, floors, plumbing, toilets, lighting, changerooms, disability access, storerooms, and water fountains. Confirm management and OHS controls including procedures, signage, inductions, qualifications, evacuation plans, manual handling, electrical and chemical safety, and MSDS. Verify first aid readiness, including stocked kits and defibrillator status. Cover gym-specific items like machine condition and maintenance logs, floor tiles and trip hazards, FOB alert checks, sauna and heat box safety where applicable, out of order signage, and pre-activity declarations.
