This field observation sheet is designed for pest control operations and branch-level training. It captures customer and site details, premises type and treated areas, and documents pest infestations, including German, American, and Brown Banded cockroaches. Inspectors can note signs of activity, hotspots and hideouts, damages observed, and hygiene and sanitation conditions.
