Use this checklist to complete and document health and safety induction for employees and volunteers. Capture site, date, names, and location; confirm awareness of rights and responsibilities, PPE, training, hazard and incident reporting, emergency procedures, and key facilities. Verify knowledge of fire extinguishers, wardens, first aiders, and kit locations. Include bullying and harassment policy, injury management program, and hazard register review. Reference fact sheets for computer use, ladders, manual handling, and equipment competency. Attach driver license photo, acknowledge receipt of the Safety Induction Manual, and sign off.

This health and safety induction checklist helps standardize onboarding for employees and contractors on any worksite. It covers key topics including alarms and evacuation procedures; locations of fire extinguishers, hoses, blankets, first aid kits, defibs, and first aiders; site speed limits and parking; site rules such as smoking, exclusion zones, and PPE; incident, near-miss, and hazard reporting processes; hazard registers and controls; hazardous substances with inventories and SDS; competency assessments for SOPs/JSAs; where and when PPE must be worn and stored; equipment training, fit testing, and replacement schedules; the organization’s health and safety policy and manual; and worker participation through engagement, toolbox meetings, roles, and responsibilities. Use this template to ensure every inductee receives consistent safety information and acknowledges their responsibilities.
