This hazard observation card template helps teams quickly record unsafe conditions and behaviors observed on site. Capture key details like site, date, observer, location, and personnel, then document hazards such as trip or slip risks, housekeeping issues, and equipment concerns. Track unsafe behaviors including body position, following safe procedures, eyes on task, and mind on task. Include notes, assign follow-up actions, and obtain the observer’s signature to complete the record.
