This monthly inspection template helps underground mining teams verify the readiness of Safe Havens (refuge stations). It covers access signage and illumination, housekeeping and cleanliness, seating and lighting, toilets, and proximity to stretchers. Inspect first aid provisions, AED and Entonox, spare self-rescuers, emergency water, temperature monitoring, and fresh air systems. Confirm communications, including telephone and DAC, availability of logbook and writing materials, and posted SOPs and fire and rescue plans. Record issues, assign responsibility and due dates, and capture signatures to confirm Safe Haven readiness and compliance.
