Use this equipment damage, loss and theft report form to document incidents quickly and consistently. Capture the incident type, work being carried out, people involved, equipment details and identification numbers, location, and a complete description of how the damage, loss or theft occurred. Attach photos, record estimated repair or replacement costs, and note the responsible person. If theft is involved, add police file and station numbers and the officer in charge. Collect employee and supervisor signatures, then export the finished report to PDF, Word, Excel or share via web link.
