Working with Hazardous Substances COSHH Assessment Form

This COSHH assessment form helps teams working with chemicals and other hazardous substances identify risks and put effective controls in place. Capture chemical names, verify current Safety Data Sheets, and record GHS hazard classes and H codes. Select appropriate PPE across eye, respiratory, hand, hearing, and protective clothing. Document storage requirements, spill response, firefighting media and considerations, and first aid actions for skin, ingestion, inhalation, and eyes. Evaluate risk by application method, amount used, exposure time, and who is exposed, then set a task hazard level and sign off to confirm controls.
