This Layered Process Audit template helps evaluate safe interactions between powered industrial trucks (PITs) and pedestrians in manufacturing and warehouse environments. It covers operator seatbelt use, maintaining an unobstructed view, and functioning safety alerts such as horns, backup alarms, blue lights, and red halo lights. Checks include horn use at intersections, full stops for pedestrians, and pedestrians announcing their presence and keeping a minimum 3 ft distance until loads are lowered and PITs are off. It verifies completion of preshift inspections, controlled key issuance, and proper parking with forks down and keys removed. The template also promotes use of designated pedestrian aisles, scanning dome mirrors at blind spots, safe spotter positioning (10 ft away with clear communication), and no cell phone use in aisleways. It includes placement of SQDCI boards away from high-traffic areas to reduce risk.
