Powered Industrial Truck Operator Performance Evaluation

This template supports evaluation of forklift and powered industrial truck operators. It covers daily inspections, proper PPE, assessment of work areas and hazards, operating in weather conditions, seat belt use, traffic and intersection behavior, pedestrian yielding, maneuvering within aisles, correct load approach and lifting, travel with load at proper height, smooth lowering and stopping, load balance and fork placement, use of approved containers, use of bridge plates and ramps, stacking within designated areas, weight checks, and safe parking with keys removed. It also includes verification of training certification and renewal dates, and a rating system with reevaluation intervals to guide authorization, mentoring, or additional training.
