PCS Card Safety Observation Checklist

This PCS card template supports behavior based safety observations in industrial workplaces. Record observer and observee details, location, and task, then review PPE use and correct tools. Assess safety controls including identification of hazardous energy, electrical work practices, suspended load avoidance, machine guarding, lockout tagout, hot work compliance, forklift operation, pre use equipment checks, loading and unloading procedures, working at height, and following task specific SOPs. Evaluate body positioning and ergonomics, maintain three points of contact when climbing, and document comments from both observer and observee. Capture the safety conversation, agreements on corrective and preventive actions, and any follow up needed to address deviations.
