Use this HSE COSHH assessment template to document and control risks from hazardous substances at work. List chemicals and intended use, who may be exposed, hazard properties, and quantities. Record Workplace Exposure Limits (WELs) and detailed emergency procedures for eye and skin contact, inhalation, ingestion, and spills. Specify prevention and control measures including engineering controls, access restrictions, SOPs, and approved PPE. Capture disposal, handling and storage requirements, training needs, and overall risk assessment. Obtain supervisor authorization and signatures to validate the assessment.
