This Good Catch Report template helps teams capture and act on safety observations and near misses before they become incidents. Document severity using Red, Yellow, or Green levels, record the specific area, time, and date, and provide a clear, concise description of the event. Note whether Company Safety was informed and, if desired, include who reported it. Add lessons learned and actions taken to drive continuous improvement. For Red-level events, root cause analysis is required. Use this template to strengthen hazard awareness and prevent future occurrences.
