Use this CATS Report template to document workplace safety observations. Capture initial descriptions of acts observed, mark them as safe or unsafe, and categorize by PPE, tools and equipment, housekeeping, behavior, procedural, or training issues. Record detailed notes on PPE items such as head, eyes, ears, respiratory, hands, feet, harness or lanyard, and hi vis. Assess procedures, risk assessment, and safe systems of work, and note remedial actions with follow-up to improve workplace safety.
