Use this Therapy Dog Suitability Assessment to evaluate whether a prospective therapy dog and handler are ready for service. The checklist covers identification and vaccinations, basic health and grooming, temperament and acceptance of strangers, obedience commands, leash walking beside the handler, behavior in crowds and public places, and reactions to trolleys, automatic doors, bins, dropped objects, and other dogs. It includes sections for toileting, being left briefly without the handler, in-office and outdoor assessments, a brief video, and assessor notes with signature and date.
