Restraints Checklist for Patient Safety

This Restraints Checklist helps hospital and long-term care teams evaluate and document the safe use of patient restraints. Verify there is a valid physician order in the Meditech system, record the date and time restraints were initiated, and document status checks at least every two hours. Confirm there are no signs of injury, ensure a two finger breadth fit, and check that restraints are secured to an immovable part of the bed. Use this template to support patient safety, clinical compliance, and consistent documentation.
