This site survey template supports planning and documenting Parity Medical E-Series E-Access lock installations in healthcare environments. It captures trust, site and contact details; verifies staff involvement and follow ups; and requests a short video of proposed install areas. The gateway section covers Cassia Gateway location, WiFi or LAN with PoE, power socket availability, data outlet ID, cable length and SSID. The locks section records furniture type, photos, orientation, condition, existing lock type, materials, thickness and any obstructions. USB card reader checks include client PC location, asset number, available USB ports or hub and mounting. It prompts NHS ID card testing, access type selection and provides final sign off checks for housekeeping, surveyed locations, loading and parking.
