Use this medical observation template to support emergency and clinical care. Capture safety checks, vital signs, AVPU, oxygen saturation, pulse, blood pressure, temperature, blood glucose, pain scores, and notes. Document SAMPLE history, Glasgow Coma Scale, and a concise drugs formulary for common presentations such as anaphylaxis, asthma, chest pain, seizure, and hypoglycaemia. Record drugs administered, treatments provided, and clear handover details to ensure continuity of care.

Sepsis 6 Checklist
This Sepsis 6 checklist supports clinicians in the early recognition and initial management of suspected sepsis. It aligns with common clinical practices such as ABCDE assessment and NEWS escalation, and guides the six key actions often performed within the first hour: deliver high flow oxygen, obtain venous access and blood cultures, check venous blood gas and lactate and haemoglobin, start broad spectrum antibiotics per local guidelines, give crystalloid fluid bolus, and monitor urine output hourly. It includes prompts for escalating care based on National Early Warning Score thresholds and encourages timely communication with senior clinicians using SBAR.
