Use the Morse Fall Risk Assessment, adapted from the Morse Fall Scale, to quickly evaluate a patient’s likelihood of falling. The checklist guides clinicians through six factors: recent fall history, secondary diagnosis, ambulatory aid, IV or heparin lock, gait or transfer status, and mental status. Scores roll up to an overall Low, Medium, or High risk rating, helping nurses prioritize interventions and document patient safety. Includes fields for patient and nurse details, date and location, and signature capture, with a reference table for interpreting scores.
