ICRA Assessment for Healthcare Construction

This Infection Control Risk Assessment (ICRA) template supports safe healthcare construction and renovation. It captures key project details, affected departments, and maps construction activities (Types A–D) to patient risk groups to determine the appropriate ICRA class (I–IV). The checklist outlines required controls for each class, including HVAC isolation, sealing penetrations, dust mats, HEPA filtration, negative air pressure, barrier installation, PPE and shoe covers, controlled waste handling, HEPA vacuuming, and disinfectant cleaning, plus inspection and sign-off by Infection Prevention. It also prompts assessment of additional risks such as noise, vibration, air quality, utility failures, and operational impacts, with approvals and signatories from Facilities Management, Infection Prevention, and the contractor.
