Executive Patient Safety Rounds Checklist

Use this executive patient safety rounds checklist to support hospital leaders during routine rounding. Capture when and where rounds occur, who prepared the report, and the department or unit visited. Prompt discussion around the learning board, current safety concerns, and recognition for strong patient safety, patient experience, and teamwork. Record the roles of staff interviewed, including physicians, nurses, nursing assistants, unit clerks, pharmacists, respiratory therapy, physical therapy, nutrition, case managers, environmental services, technologists, and others. Track shift or day of week and document executive involvement, such as the chief executive officer, chief financial officer, chief operating officer, chief nursing officer, patient safety officer, or other leaders. Standardize leadership engagement and follow up actions to improve patient outcomes and safety culture.
