EHS Audit Form for Radiotherapy Sites (Italy)

This EHS internal audit form supports audits at radiotherapy hospital sites in Italy. It covers verification of required health and safety documentation such as PSC, POS, DUVRI and radioprotection coordination records. Inspect site conditions, access controls and safety signage in controlled areas and bunkers. Assess interference risks with other contractors. Evaluate field engineer readiness, including updated procedures, lockout tagout, tool condition, PPE and dosimeter use, and awareness of emergency procedures. Review task risks across electrical, ladder, chemical, mechanical, radiation and biological hazards, including disinfectant use and SDS access. Check environmental and waste management practices, including used lead batteries and SF6 storage. Verify company car safety equipment and first aid kit where applicable. Conclude with training effectiveness checks, findings, corrective actions and signatures.
