This hand hygiene audit helps healthcare teams assess staff compliance with preparation, the WHO Five Moments, and correct technique. It covers bare below the elbows, short unpolished nails, and covered cuts. Observers check that hands are cleaned before patient contact, before clean or aseptic tasks, after body fluid exposure, after touching a patient, and after touching patient surroundings.
