Use this utility locate checklist to document 811 locate requests and field verification. Capture ticket number, preparer, date, and job location; confirm whether the locate was completed by the work date; note work performed; and verify that the 811 positive response has been updated. Attach north south and east west photos, mark completion status, indicate if this is a re-mark, and record whether the locate was finished on time with an explanation when required.
