This Hot Work Permit template helps control risks during welding, cutting, and other ignition-producing tasks. Record permit start and end times, location, and equipment details; confirm JSA completion and relocation or protection of combustibles; verify energy isolation and ignition source controls; assign a knowledgeable firewatch; and document required precautions such as grounding, fire extinguishers, lockout/tagout, SDS review, fall protection, non-sparking tools, and emergency access. Capture atmospheric testing for H2S, CO, oxygen, and LEL with device and calibration details, and obtain signatures from the worker, firewatch, HSE, and company representative.
