Behavior Based Safety Observation

This Behavior-Based Safety Observation template helps teams document tours and conversations, record observed conditions, and agree on corrective actions. It includes observer and leader details, job roles such as drill, deck, marine, and maintenance crews, and locations like rig floor, pipe deck, machinery space, and accommodations. Capture behaviors and hazards including housekeeping, tools and equipment, electrical, pressure, struck-by, caught-between, fire or explosion, PPE, impairment, dropped objects, loss of containment, falling, marine integrity, well control, and environmental risks. Note procedural gaps not known, not followed, not understood, or inadequate and certify when work is safe to restart.
