Workplace Risk Assessment Template

This HSE based workplace risk assessment template helps employers, owners, and managers plan and document a structured assessment of hazards at work. Define the goal, scope, location, timing, and participants; capture hazards with descriptions and photos; assess who may be harmed, likelihood, and consequences; and determine risk levels using a risk matrix. Record existing controls, develop additional control measures, create and assign actions with due dates, and decide whether hazards can be eliminated, controlled, or monitored. Conclude with a review plan that sets how effectiveness will be monitored and how often the assessment will be reviewed, with guidance to review at least annually. Includes sign off for responsible persons.
