This task-based risk assessment template helps teams identify hazards for specific activities, evaluate who may be exposed, and determine likelihood and severity using a risk matrix. Capture site, activity, issue number, and review purpose, then add hazards such as electricity, moving machinery, manual handling, DSE, COSHH, noise, and work at height. Record existing controls, assign risk ratings, and plan additional measures. Specify required PPE and systems of work, including hearing, eye, respiratory, hand, body, hi vis, and footwear protection with applicable standards, plus permits and machinery guards. Conclude with manager sign off and communication to all relevant personnel.
