Worker Training Record and Competency Assessment

This worker training record template supports planned task observations and competency assessments. It guides evaluators through confirming the presence of a safe operating procedure, verifying that training has been delivered and understood, and checking correct selection and use of PPE such as boots, glasses, harnesses, and hard hats. The form captures adherence to procedures, tool and equipment suitability, housekeeping, demarcation, and common risk scenarios including caught in or between, overexertion, burns, insufficient air, struck by, unsafe positions, and falls. It includes space to note controls, schedule manual handling training, recommend new or modified SOPs, equipment repairs, layout changes, new safety rules, retraining, or ergonomic studies. The record concludes with competency determination, comments, and signatures for the observer, site supervisor, and the person observed.
