This Project Safety Assessment template helps project teams plan and document site safety before work begins. It covers project setup details, site access, parking and break areas, and emergency contacts for hospitals, clinics, security, and management. Define scope of work and link appropriate JHAs for activities such as cable decommissioning, cable tray and conduit installation, fiber installation and splicing, labeling and pulling fiber, and OSP confined space. Identify high risk activities including lift use, confined space, ladders, electrical work and LOTO, scaffolding, silica, asbestos, traffic control, heavy machinery, and hot work. Capture orientation and training requirements, certifications, internal training, and designated trainers. Specify PPE needs such as helmets, cut resistant gloves, safety glasses and footwear, high visibility vests, hearing and fall protection, plus any specialty PPE.
