Southern USI Site Condition and Maintenance Report

This template supports site condition assessments and maintenance reporting for Southern USI locations. Inspect site number and location details, note whether tracking is required, record site size, ratchets and bracing needs, and document access issues such as gates, locks, or keys. Capture defects on fixed ladders and catwalks including missing hatches, broken welds, or loose rungs. Add detailed notes, attach photos for evidence, and compile other issues discovered on site. Conclude with sign offs from the team leader and the assessor to confirm findings and next steps. Use this report to communicate issues to management for timely remediation and safer operations.
