Site Specific Hazard Assessment Checklist

Use this site specific hazard assessment template to perform a daily pre job review of hazards and controls before work begins. Capture job details, site location, personnel, and tasks. Identify common risks such as electricity, energized equipment, excavations, underground services, moving machinery, traffic, noise, weather, working at heights, confined spaces, hot work, welding and cutting, gas exposure, heat or cold stress, slippery conditions, steep or uneven surfaces, wildlife, and working alone. Plan controls including PPE, barricades, signage, spotters, fall protection, gas monitoring and bump tests, communication devices and check in procedures, lockout tag procedures, machine guarding, ventilation, fire extinguishers, first aid, ladders and scaffolds, and permits. Verify readiness, document inspections, and record who conducted the assessment and subcontractors involved.
