Use this Point of Work Safety Assessment template to complete a dynamic risk check before starting tasks on site. Verify RAMS and method statements have been read, confirm PPE, plant certifications and operator competency, and check permits to work. Identify hazards such as working at height, lifting, electricity, traffic, hazardous substances, excavations and confined spaces, then document required control measures. Record relevant toolbox talks, attach lift plans and inspections, and capture sign-off from the site supervisor. Revalidate if the task remains unchanged for up to one week.
