This Permit to Dig (PTD) closure form helps teams formally close out ground-breaking works. Record site location and job reference, declare whether any incidents with underground services occurred, confirm that excavations are backfilled and surfaces reinstated, and verify that the area is left safe and clear of personnel and equipment.
